- Valid government-issued photo ID: passport, driver's license or state ID.
- The documents for your procedure, unsigned: you must sign in front of the notary.
- Full details of the people involved, for example the agent in a power of attorney.
- The country where the document will be used: if it is outside the U.S., it may need an apostille.

Is my appointment confirmed when I submit the form?
